Administrative Service Coordinator and Forensic Liaison
WORK SCHEDULE: The normal workweek for this position is 37.5 hours each week. 8:30 am to 5:00pm each week. The schedule may be adjusted to meet the need of the consumer.

DESCRIPTION OF ESSENTIAL RESPONSIBILITIES AND DUTIES:
- Contact and schedule an evaluation with Administrative Service Coordination consumer referrals within Emergent, Urgent and Routine access standards. Following the assessment of a new referral, develop and implement a plan of action, making recommendations and linking to appropriate levels of treatment and support with adherence to appropriate access standards.
- Actively assist consumers in locating, linking, and problem solving with internal and external services requested to meet the individuals recovery needs.
- Monitor the consumer’s receipt, participation and effectiveness of service delivery as evidenced by documentation at the time of service or at minimum a monthly case management progress note. Utilizing a professionally written format, enter all information into the Qualifacts system daily or within 3 days of the service.
- Advocate for incarcerated consumers and monitor the Forensic system services evidenced by monthly jail site visits. Including dialogue with the Allegheny County Jail social work staff, ACDHS Forensic staff, probation/parole staff, local magistrates and or police departments
- Monitor, update, and maintain the TCV/County CHIPP list and fax updates to the Allegheny County DHS by the 10th of each month.
- Fiscal responsibility to the Agency and to SCU productivity expectations of 1250 hours per year.
- Provide information and referral, telephone consultation and crisis intervention to consumers requesting services as intake support.
- Work closely with community resources and maintain informal support network building through community and natural supports.

REQUIRED MINIMUM TRAINING: Bachelor's degree with major source work in sociology, social work, psychology, or related social sciences with two years of experience in related field.

Drug and Alcohol Intervention Specialist
One (1) Contract Part Time to Full Time Position for Drug and Alcohol Intervention Specialist for the Alternatives D&A program at the Turtle Creek Sites.

Job Objective: To expand community outreach efforts by way of pre-treatment activities to improve engagement, retention and provide education and risk assessments to chemically dependent consumers being evaluated for D&A Treatment Services. Act as liaison between TCV and the criminal justice system and other supports & services/individuals in the community. One contract position, part time to full time, will be located at the Turtle Creek Site.

DESCRIPTION OF ESSENTIAL RESPONSIBILITIES AND DUTIES:
- Provide D&A Risk Assessments to consumers opened to D&A Intervention Services.
- Act as liaison between D&A consumers, the criminal justice system, probation and parole officers and case managers at work release programs. As liaison the D&A Intervention Specialist will be required to travel for linking, advocacy and outreach services.
- Provide intensive outreach to engage and retain consumers in D&A treatment, including phone calls and face to face services, and provide follow up for cancellations and no shows.
- Identify barriers that prevent utilization and attendance of D&A Treatment services.
- Act as liaison with case managers at the Allegheny County Jail.

REQUIRED MINIMUM TRAINING: Minimum Bachelor’s Degree in Human Services Field plus two years experience working with chemical dependency and the criminal justice system. Computer skills, excellent oral and written communications skills and ability to travel in Allegheny County to provide intervention services. Knowledge of D&A risk assessment.

Enhanced Clinical Service Coordination– Adult Service Coordinator
PRIMARY PURPOSE OF THIS POSITION: Coordinate the provision of support services for adults with serious and persistent mental illness who need assistance in accessing, coordinating and monitoring resources and service.

WORK SCHEDULE: The normal work week (37.5 hours) for this position is Monday – Friday from 9 AM to 5 PM, with a flexible schedule to provide evening and weekend appointments when necessary and/or to deal with a crisis or other unusual situations as needed.

DESCRIPTION OF ESSENTIAL RESPONSIBILITIES AND DUTIES:
- Assess consumer needs through utilization of a strengths assessment, Global Assessment of Functioning Scale, consumer and/or collateral interviews and review of clinical and social information.
- Coordinate and facilitate the development of a comprehensive Individualized Service Plan, provide input into the plan and collaborate with members of the Enhanced Clinical Service Coordination Team to ensure that goals are mutually supportive.
- Monitor all services received, review cases, meet with consumer’s family and significant others.
- Attend weekly Enhanced Clinical Service Coordination team meetings.
- Ensure consumer’s needs are met through utilization of natural supports (family, friend), community and generic services and specialized services.
- Investigate new resources and communicate with providers of prospective resources as a liaison and advocate on behalf of the consumer.
- Attend to state, county and agency regulations governing Enhanced Clinical Service Coordination
- Advocates for the consumer to ensure responsiveness from natural, community, generic and specialized services.
- Ensure face-to-face contact with the consumer to build strengths, monitor and achieve goals.
- Provide 24-hour crisis services as scheduled for Enhanced Clinical Service Coordination consumers, handling both face-to-face and telephone requests for crisis intervention.

REQUIRED MINIMUM TRAINING: A Bachelor’s degree with major source work in sociology, social work, psychology, or other related social sciences, with 5 years experience in related field. Must have a valid PA driver’s license with a good driving record and proof of insurance. Must be willing to transport consumers using personal or Agency vehicles and use their vehicle for work related duties.

Intake/Administrative Service Coordinator
PRIMARY PURPOSE OF THIS POSITION: The primary purposes of this position is to function as a team member within the Service Coordination Unit, other programs within the agency, community and institutional resources as well as provide crisis intervention, information and referral and timely access to services. Utilization of these services will present the opportunity for consumers to move towards a more stable, meaningful and productive life.

WORK SCHEDULE: The normal workweek for this position is 37.5 hours from 8:30 am to 5:00pm each week. The schedule may be adjusted to meet the need of the consumers.

DESCRIPTION OF ESSENTIAL RESPONSIBILITIES AND DUTIES:
- Obtain information from individuals and referral sources requesting services/treatment as well as complete a comprehensive screening for assessment and/or referral using best practice standards with accuracy.
- Provide telephone and walk-in emergency and crisis intervention services as needed. This includes assessing the degree and seriousness of disturbance of the presenting consumer and providing appropriate intervention.
- Coordinate outpatient referrals to ensure compliance with contract requirements and the access standards with accuracy.
- Contact and schedule an evaluation with Administrative Service Coordination consumer referrals within Emergent, Urgent and Routine access standards. Maintenance Foreman
PRIMARY PURPOSE OF THIS POSITION: The primary purpose of this position is to function as a team member within the maintenance department and with all staff to meet the primary purpose of the department. This highly skilled, foreman position provides maintenance and repair work and coordination and supervision of same in the construction, mechanical, plumbing, HVAC, or electrical trades.

WORK SCHEDULE: The normal work-week for this position is 37.5 hours each week. Position hours are usually from 8:00AM to 4:00 PM. Regular call outs may be necessary, and may be on call 24/7.

DESCRIPTION OF ESSENTIAL RESPONSIBILITIES AND DUTIES:
- In conjunction with the Facility Manager, assure all buildings/spaces/grounds rented or owned by Turtle Creek Valley MH/MR Inc. are maintained to current building code, program code and Agency standards.
- Provide on call coverage
- Execute agency wide preventative maintenance plan for: HVAC filter cleaning/replacements, motor oiling, condenser cleaning, floor stripping & finish, facility painting, carpet cleaning, bulb replacement, battery/bulb replacement for smoke/CO detectors & emergency lighting, carpet cleaning/replacement, roof repair and resurfacing, gutter cleaning.
- Review all work orders for appropriateness of request, either referring back to requestor with further instructions and vendor suggestions or prioritizing and delegating to be completed by maintenance staff.
- Ensure the quality/quantity of work of maintenance and housekeeping staff.
- Inspect and diagnose problems and determine the best way to handle them.
- Display fiscal responsibility to the agency by properly using agency property, charge accounts, and obtaining supplies from vendors.
- Maintain diplomatic working relationships with all landlords, supervisors and staff.
- Provide on the job training or other job development to staff as needed
- Repair and maintain buildings, mechanical equipment, work on plumbing, electrical, and some air conditioning and heating systems.
- Remove or install carpeting, tile or other flooring. Move heavy furniture and equipment. Install machinery, furniture, locks, fixtures, etc. Strip wax from tile floors and resurface the same.
- Build partitions, make plaster or drywall repairs, fix, remodel, paint all parts of building structures
- Performs outdoor grounds maintenance including grass cutting, pruning, shrub and tree removal, snow and ice removal.
- Maintain clean, safe workplace, mindful of other employees’ and clients’ safety and working/living needs.

KNOWLEDGE, SKILLS AND ABILITIES, TRAINING AND EXPERIENCE REQUIREMENTS:
- Knowledge of Building codes, OSHA requirements, blueprint reading, Building repair, maintenance, engines, electrical, plumbing, HVAC standards and repairs.
- Ability to communicate effectively both orally and in writing with staff, consumers and the general public. Must be able to follow standard operating procedures, and to perform duties in a professional manner and appearance.
- Must be able to read, write, complete simple reports
- Must know, understand and make decisions according to Agency Mission, Vision, and Values and within proper policy and procedures.
- Must use good time management skills.
- Ability to work weekends, shifts, early starts, and on-call time.
- Ability to take and follow directions and delegate appropriately.

- Ability to complete multiple tasks, prioritize work orders at multiple locations. - Ability to lift heavy loads, multiple times, push, pull, climb ladders, and to otherwise consistently perform physically challenging work.

REQUIRED MINIMUM TRAINING: Must have High school diploma or equivalent and four years of relevant experience. Must have valid PA drivers license, and vehicle that can transport tools between various sites; access to own tools a plus. Must be physically fit and able to repeatedly carry heavy loads. Act 33/34 clearances.

Psychiatrist
PRIMARY PURPOSE OF DEPARTMENT:
The primary function of Psychiatric Services is to provide best practice psychiatric services including initial assessments and all other treatment to consumers as well as to function as part of the treatment/planning team. This is done in accordance with all regulatory requirements as well as the Agency’s Vision, Mission and Values.

PRIMARY PURPOSE OF THIS POSITION:
The primary purpose of this position is to provide quality psychiatric service to all consumers and to function as a team member with therapists, nurses, service coordinators, support staff, administrators and other psychiatrists.

WORK SCHEDULE:
The normal work-week for this position is 37.5 hours; hours may vary depending on program needs.

DESCRIPTION OF ESSENTIAL RESPONSIBILITIES AND DUTIES:
- Perform psychiatric evaluations/services in accordance with best practice standards of care and according to all regulations
- Provide medication monitoring visits
- Participate in providing emergency services for consumers when necessary - Provide all services based on recovery principles
- Complete all clinical paperwork (electronic records) promptly and in compliance with licensing regulations and agency policy
- Consult with staff as needed regarding treatment decisions, safety concerns
- Provide direct educational/coaching to staff formally or informally
- Provide education and consultation with consumers and family members when appropriate.
- Maintain working knowledge of up to date treatments through consultation, continuing education and other training programs

KNOWLEDGE, SKILLS AND ABILITIES: Board certified or board eligible in psychiatry. License to practice in Pennsylvania. DEA license. Experience in community psychiatry Ability to work effectively with individuals with mental illness and/or substance abuse issues in a community setting is essential. The psychiatrist needs the ability to function in a professional and caring manner when dealing with consumers and staff. Ability to use electronic medical record is required for this position. Ability to complete chart notes in a thorough and timely fashion, as well as ability to complete other required paperwork is essential.

REQUIRED MINIMUM TRAINING: Completion of a three year residence or training program in psychiatry which meets the certification requirements of the American Board of Psychiatry and Neurology or the American Osteopathic Board of Neurology and Psychiatry. Board certification/or eligible and Licensed to practice medicine in the state of PA as issued by the State board of Medical Education and Licensure or the State Board of Osteopathic Examiners

Therapist(D&A/MH Adult Outpatient Treatment)
PRIMARY PURPOSE OF THIS POSITION: The primary purpose of this position is to provide group and individual treatment for persons with D&A issues.

DESCRIPTION OF ESSENTIAL RESPONSIBILITIES AND DUTIES:
- Provides appropriate and necessary therapeutic group and individual treatment that reflect best practices.
