Child and Adolescent Service Coordinator PRIMARY PURPOSE OF THIS POSITION: Provides supportive services (i.e. linking with necessary programs, benefits, medical and psychiatric care, as well as daily living needs), coordinates these services, and monitors the services that mental health consumers in the program receive. Advocates for consumers as needed. Provides a full range of Service Coordination services to a maximum of 25. WORK SCHEDULE: The normal work week is 37.5 hours. This requires flextime as well as occasional evenings and weekend hours. DESCRIPTION OF ESSENTIAL RESPONSIBILITIES AND DUTIES: - Assess consumer needs through utilization of an assessment tool (TCM Environmental Matrix & Global Assessment of Functioning Scale), client and/or collateral interviews and review of clinical and social information. - Monitors all services received by individuals who are served by the Child & Adolescent Service Coordination program. Establishes and maintains effective working relationships with treatment/service team, other resources, consumers and their significant others. Ensures consumer needs are met through utilization of natural supports (family, friends), community and generic services, and specialized services (MH/MR, CYF, Juvenile Justice). Assists consumers to identify, link, access and coordinate such resources. - Advocates and empowers consumer as appropriate. Provides 24-hour crisis services as needed. - Coordinates and facilitates the development of the integrated Individualized Service Plan, provides input into plan and collaborates with provider-team to ensure that goals are mutually supportive, meet consumer needs and are consistent with the agency mission and professional best practices. - Ability to lead interagency treatment team meetings and be part of educational planning meetings with school district collaboration. - Documents involvement in hospital admission decisions. Documents involvement with consumer during inpatient stays and coordinates aftercare appointments within 7 days of discharge post inpatient. Ensures that consumers who are on medication are seen for medication management within 30 days post inpatient discharge. Provides 6.5 contacts within 30 days post inpatient discharge. - Face-to-face contact with assigned consumers must be made at least once a month. REQUIRED TRAINING: A Master’s degree plus one year experience in a related field or course of study, or a Bachelor’s degree plus three years experience with major course work in sociology, social work, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling or education plus two years experience working with children and adolescents. Decision Support Center Peer Support/Recovery Facilitator WORK SCHEDULE: The normal work week for this position is 20 + hours each week. The schedule may be adjusted to meet the needs of the service. GENERAL DESCRIPTION: : This position provides services to consumers participating in the decision support center within the outpatient medication clinic. This innovation in the field of psychiatry provides peer support to people before medical appointments at the first site in the state of Pennsylvania. This position reports to the coordinator of the decision support center. This new initiative is designed to provide a recovery focused, shared decision making process utilizing computer assisted technologies and peer support within the existing medication clinic. Intake/Service Coordinator PRIMARY PURPOSE OF THIS POSITION: The primary purposes of this position is to function as a team member within the Service Coordination Unit, other programs within the agency, community and institutional resources as well as provide crisis intervention, information and referral and timely access to services. Utilization of these services will present the opportunity for consumers to move towards a more stable, meaningful and productive life. DESCRIPTION OF DUTIES: - Contact and schedule an evaluation with new Administrative Service Coordination referrals within Emergent, Urgent and Routine access standards. - Following the assessment of a new referral, develop and implement a plan of action, making recommendations and linking to appropriate levels of treatment and support with adherence to appropriate access standards. - Actively assist consumers in locating, linking, and problem solving with internal and external services requested to meet the individuals recovery needs. - Monitor the consumer’s receipt, participation and effectiveness of service delivery as evidenced by documentation at the time of service or at minimum a monthly case management progress note. - Utilizing a professionally written format, enter all information into the Qualifacts system daily or within 24 hours of the service. Documentation including but not limited to Point of Entry, CCBHO logs, daily logs, Point of Entry/ Service Coordination Progress Notes, Memo to Chart and needed referrals. - Fiscal responsibility to the Agency and to SCU productivity expectations of 875 hours per year. - Obtain information from individuals and referral sources requesting services/treatment as well as complete a comprehensive screening for assessment and/or referral using best practice standards with accuracy. - Provide telephone and walk-in emergency and crisis intervention services as needed. This includes assessing the degree and seriousness of disturbance of the presenting consumer and providing appropriate intervention. - Display effective oral and written communication regarding consumers needs and make appropriate recommendations for the level of treatment with accuracy. - Coordinate outpatient referrals to ensure compliance with contract requirements and the access standards with accuracy. Mobile In Home Psychiatric Nurse PRIMARY PURPOSE OF THIS POSITION: Provides nursing and supportive services to persons with chronic mental illness. Works with psychiatrists, therapists, case managers, primary care physicians, and other agency staff in coordinated treatment planning and medication monitoring. Targeted Case Manager PRIMARY PURPOSE OF POSITION: The primary purpose of this position is to function as a team member with clinical modalities and to coordinate the provision of supportive services for adults with serious and persistent mental illness who need assistance in accessing, coordinating and monitoring resources and services.
